Welcome to Evergreen Pediatric Clinic! Please bring your insurance information and available copies of medical and vaccine records to your first visit. You may download new patient and registration paperwork to fax or mail in advice, or bring completed for your first visit. Please take a moment to check out our extended office hours, our well child check visit schedule, and information about phone nurse advice. We look forward to meeting your and caring for your children!
